Before participating in the program, we kindly ask you to review and complete the following consent form. Participants will engage in the following activities under the guidance of experienced instructors: Handling hot wax and candle molds, Using tools (e.g., thermometers, stirring implements), Adding scents and colors to candles, Working with a variety of candle-making materials, Potential Risks: While all safety precautions will be taken, participants will be working with hot wax and tools, which may pose some risks. These risks include, but are not limited to: Minor burns from hot wax, Potential allergic reactions to fragrances or other materials, All participants will receive instructions on safety procedures and the proper use of equipment. Instructors will be available to assist throughout the process. Consent and Release: By signing below, I acknowledge and agree to the following: I understand the nature of the Candle-Making Workshop and the activities involved. I understand the potential risks associated with the use of hot wax and related materials. I agree to follow all safety instructions provided by the instructors. I release Chrisanthi Demopoulos, Earth Wicks and its instructors from any liability for injuries or damages that may occur during the workshop, except in cases of negligence or willful misconduct. I consent to [Participant’s Name] participating in this workshop and understand that I/we assume full responsibility for any risks associated with the activity.
